ATHLETIC CLEARANCE

XC Athletic Clearance Procedure

• NOTE: the confirmation form at the end of the Home Campus online process will need to be printed and signatures (details below) completed in order to be cleared. 

1. Schedule a physical exam appointment with your doctor ASAP. Print and take a copy of this Physical Exam Form* with you. A doctor's physical that was completed within the last year can be used in lieu of a new physical, but you will need the form from that prior visit. If that physical expires (it is good for one year from the date of the visit) midseason, an updated physical will be needed.

   • *Note: Iron (specifically "ferritin") is crucial for endurance athletes and is a very common deficiency in teenagers and runners. Low ferritin levels can lead to fatigue, irritability, dizziness, and decreased performance/recovery. Runners need a higher ferritin level than the general population (source). For these reasons, we highly suggest asking your doctor about a "ferritin" blood test in addition to the standard physical. Ask specifically for "ferritin", not just iron.

   • Ferritin levels for runners should exceed 40ng/mL-- be sure to check the number in the test result as runner-normal is different than standard-normal, so it may not be flagged.

   • If ferritin is below 40ng/mL ask for your doctor's guidance on supplementation and/or treatment.

2. Log in or create an account on: https://www.homecampus.com/login

   • School: Poly/Long Beach

   • Sports: Cross Country, Track & Field

(and any other sports you will be participating in at Poly)

2. • Year: 2025-26

3. Complete profile on Home Campus and e-sign all forms.

   • MUST include personal insurance information

   • MUST upload a photo/scanned copy of Physical Exam documents

4. Print Confirmation Form from Home Campus

   • Three signatures are required:

     • Parent/Guardian

     • School Nurse

       • Nurse's office is located next to the pool entrance behind 100 building (main building at the east end of the quad)

       • Nurse will require a paper copy of Physical Exam Form

     • Banker

       • Banker's window is on the front of the 100 building facing the quad

• Nurse and Banker will again be available once the school year starts.

5. Turn in completed Confirmation Form (with all three signatures) to a coach.

"2nd Sport" Clearance for Track Season (to be completed in the WINTER)

If you were already cleared for Cross Country season, then you can follow the steps below to complete your clearance for Track:

1. Confirm that your current physical expires after the end of track season (first week of May/end of May for CIF athletes).

   • Sports physicals are good for one year following the date of exam.

   • If your physical will expire, get an updated one before the start of track season. This can be done at a local urgent care clinic if your doctor does not schedule physicals until one year after the previous one. 

2. Log in to https://www.homecampus.com/login. 

3. Print Confirmation Form

   • Confirm that "Track & Field" shows up as a sport on your account. If it does not, click on "+Sport" to add.

   • Two signatures are required:

     • Parent/Guardian

     • School Nurse

       • Nurse's office is located next to the pool entrance behind 100 building

4. Turn in completed Confirmation Form to a coach. The new procedure is for these forms to be submitted to the athletic secretary by us.
