XC Athletic Clearance Procedure

• NOTE: the confirmation form at the end of the Home Campus online process will need to be printed and signatures completed (details below) in order to be cleared. 

1. Schedule a physical exam appointment with your doctor ASAP. Print and take a copy of this Physical Exam Form* with you. A doctor's physical that was completed within the last year can be used in lieu of a new physical, but you will need the form from that prior visit. If that physical expires (it is good for one year from the date of the visit) midseason, an updated physical will be needed.

   • *Note: Iron (specifically "ferritin") is crucial for endurance athletes and is a common deficiency in teenagers and runners. Low ferritin levels can lead to fatigue, irritability, dizziness, and decreased performance/recovery. Runners need a higher ferritin level than the general population (source). For these reasons, we highly suggest asking your doctor about a "ferritin" blood test in addition to the standard physical. Ask specifically for "ferritin", not just iron.

   • Ferritin levels for runners should exceed 40ng/mL-- be sure to check the number in the test result as "normal" for runners is different than for the average person, so it may not be flagged. If ferritin is below 40ng/mL ask for your doctor's guidance on supplementation and/or treatment.

2. Log in or create an account on: https://www.homecampus.com/login

   • School: Poly/Long Beach

   • Sports: Cross Country

(and any other sports you will be participating in at Poly throughout the school year)

2. • Year: 2026-27

3. Complete profile on Home Campus and e-sign all forms.

   • MUST include personal insurance information

   • MUST upload a photo/scanned copy of Physical Exam Form

4. Print Confirmation Form from Home Campus

   • Three signatures are required:

     • Parent/Guardian

     • School Nurse

       • Nurse's office is located next to the pool entrance behind 100 building (main building at the east end of the quad)

       • Nurse will require a paper copy of Physical Exam Form (keep a copy for your records as well!)

     • Banker

       • Banker's window is on the front of the Big Gym near the student store

   • Check this document for Nurse and Banker availability

5. Turn in your completed Confirmation Form (with all three signatures) to the Athletic Secretary, Ms. Lissette. The Athletic Office is currently located on the first floor of the 100 building but will be moving to the "Poly Plaza" bungalows for the 26-27 school year.

ALL ATHLETES MUST BE CLEARED TO COMPETE IN ANY RACE!
